  • Routine Annual Physicals for employee/spouse up to 2 visits per year;
  • Annual Mammograms; Pelvic Exams; Routine Pap Smear; TB test; Prostate Screening Test; Hemmocult test

Accidental Injury Endorsement $550 per member per benefit period, deductible & co-insurance waived.
